Your completed medical leave form can only be shared with your provider and your employer. The LeaveGuru program has access only to demographics and diagnosis codes. LeaveGuru staff does not have access to your medical records. The LeaveGuru app is 100% HIPAA compliant using the latest data storage and security features.

If LeaveGuru cannot complete your medical leave form automatically, you will receive an app notification and email requesting more information.

Simply log back into the app, update the information that needs to be changed, and resubmit your form. The updated form will automatically be faxed to your employer and provider. You can also easily download the newly completed form directly from the app.

While HR managers and employers generally mean well, be sure to provide them with this information from the Department of Labor website, along with your completed FMLA form: Employers must accept a complete and sufficient certification, regardless of the format. The employer cannot reject a certification that contains all the information needed to determine if the leave is FMLA-qualifying. The employer cannot refuse:

    • A fax or copy of the certification;
    • A certification that is not completed on the employer’s standard company form; or
    • Any other record of the medical documentation, such as a communication on the letterhead of the healthcare provider.

Acceptance of the recommendations on the medical form is subject to the rules and regulations of your employer’s leave policy.
